Lift Type
Passive Lifts:
A passive patient lift does not require the patient to participate in the lifting process. Examples of passive lifts include the Oxford Advance and Oxford Presence.
Active Lifts:
An active patient lift (or Stand-aid) requires the patient to participate in the lifting process. An example of an active lift is the Oxford Journey.
Sling Attachment
Loop Attachments:
Slings with loop attachments allow positioning adjustments to be made by selecting the different coloured loops. Examples of these slings, include the Steiss Pivot (Loops) (Loops) and Steiss Unilux slings.
Clip Attachments:
These slings utilise the Oxford Securi3 clip system for attachment to a 4-point positioning cradle. Re-positioning of the patient is achieved via the cradle's handle. An example is the Steiss Pivot (Clips).
Patient Information
Patient Ref/Location (Optional):
Insert details for your patient and their location for identification purposes (Insert name only if you have consent or are legally entitled to do so).
Weight Range
Weight Range:
Select the weight range of the patient(s) by dragging the Min and Max pointers to the appropriate position on the scale. Please be as accurate as possible as your selection will help determine the most suitable sling size.
PLEASE NOTE: Steiss Global strongly recommends a full and thorough risk assessment is conducted to ascertain the most suitable size of sling based on a patient's exact shape and measurements as opposed to their weight or height alone.
Patient Specific Details
Trunk Control:
Trunk control refers to a patient's ability to control their trunk or torso. Trunk control affects a patient's ability to hold their body upright or stable when sitting or moving.
Head Control:
Head control refers to the patient's ability to control their head. Head control affects the patient's ability to hold their head steady when sitting or moving.
FIM Score:
FIM (Functional Independence Measure) is a widely used assessment tool, particularly within the field of rehabilitation. It involves the classification of a person's mobility according to 7 levels of function, from independence to total assistance. By looking at the broad categories 'dependent' (FIM scores 3 and 4) and 'complete dependence' (FIM scores 1 and 2), FIM scoring can assist in the identification of equipment needs (remembering that this tool is not designed for person-specific risk assessment). See chart below:
Amputee:
If the patient is an amputee, please select the type of amputee from the drop-down list.
Involuntary Movement:
Confirm whether the patient is prone to involuntary movement during transfer.
Special Conditions:
Confirm whether the patient has any special conditions.
Most Frequent Task
Chair to Chair:
Transferring a patient from chair to chair. Typically, this can be from a wheelchair to a high-backed chair.
Bed to Chair:
Transferring a patient from a bed to a chair or wheelchair.
Chair to Commode/Toilet:
Transferring a patient from a chair to a commode or toilet.
Transfer to Bath:
Transferring a patient to a bath from a commode or chair.
Floor to Bed/Chair:
Transferring a patient from the floor onto a bed/chair. Typically, this could be when a patient has fallen and needs to be transferred to a bed or wheelchair, or as part of a rehabilitation programme.
Assisted Standing:
This transfer relates to assisted standing for a patient. Typically, this is achieved with an active stand-aid type lift.
